Please kindly register by using your child’s name. Each child will need a separate form.

Join us the week of June  12-June 16 from 8:30 to noon as we embark on a Cosmic VBS theme. 

Due to extensive amounts of hands on activities week of- and day of- registrations will not be taken.

Medical Release: I give persimmon for the VBS staff to administer basic first aid to the child designated to the registration in the event of an injury. I understand that the VBS staff will contact emergency services in the event of a significant injury or illness and all expenses for such emergency services will be paid by the registrant responsible party.

Photo Release: I hereby grant First Presbyterian Church permission to use photographs/videos taken at VBS of the minor designated on the registration in any manner or form for any purpose lawful at anytime. I waiver any right that I may have to inspect or approve the finished product or written…
